Tragic Middlesbrough baby born into family with history of domestic abuse and neglect dies at weeks old

The Middlesbrough-born baby’s cause of death has not yet been determined.
A tragic baby, who was born into a family with a history of abuse and neglect but fell through the cracks between social services, died at just weeks old.

It comes as recommendations are made surrounding the importance for all agencies to recognise that children who live in the area “are their full responsibility”. The independent review has highlighted “numerous failures” in handovers and communication between different council’s social care services as the family moved through the cracks for over a decade.
